Just a recap of some of the information from the parent meeting last night.
There are 3 things that have to be done for you/your child to attend YULA Invite this weekend.
1. Permission slips for YULA Invite, use the link below to access the form, print it out, and fill in the information on page two. Coach Richard or Mr Shaffer can fill in the rest.

2. USAU Medical Authorization form can be found at the link below, print it out, fill it in, and turn it in to Mr. Shaffer during the week or Coach Richard on Saturday. http://www.usaultimate.org/assets/1/Page/Medical%20Authorization-V4.pdf
3. Fall dues have to be paid or you can not attend YULA or any other tournaments until they are. The following people still owe dues: Luke Young, Kobi Evans, Tyler Paul, Andrew Lakey, Chris Clark, Dwayne Thomas, Brandon Hoshaw, Andrew Shreve, Shane Robinson, Jake Carlson

USAU Dues need to be paid and your ID number needs to be given to Coach Richard ASAP. All the remaining tournaments this year are sanctioned and you will have to be a current member to play. You can signup or login and pay here: https://play.usaultimate.org/members/login/
